Member : Georgia Planning For Healthy Babies


Great News!
The Planning for Healthy Babies (P4HB®) program is still accepting new participants. Potential participants may apply for the program online, or they may obtain a paper application at their local health department or DFCS office.

 

The P4HB® program provides no-cost family planning services to eligible women in Georgia. It also provides additional services such as primary care, substance abuse treatment, and case management including Resource Mothers Outreach to eligible women who have delivered a very low birth weight baby.


Help for Mothers Before, During and After Pregnancy
Before you become pregnant, there are things you can do to help make sure your baby is born at a healthy weight. Georgia’s Planning for Healthy Babies®? (P4HB®) program is here to help. The goal of P4HB®? To help moms be healthy and have healthy babies!

What Increases the Risk for Premature Birth?

  • Chronic health problems in the mother.
  • Smoking.
  • Inadequate weight gain during pregnancy.
  • Infections in the mother.
  • Mother’s age (under 17 and over 35).
     

What Does P4HB® Offer?

  • Health services before you become pregnant.
  • Screening for health problems and infections.
  • Help with control of chronic health conditions.
  • Health education on the needs of pregnant women.
  • Nutrition programs that encourage a healthy diet and the need for folic acid. 
  • Early prenatal care that allows health care providers to identify and treat problems early.
     

Who Is Eligible for P4HB®?
Women 18–44 years of age can join if they do not have health insurance and their family income is below a certain level. P4HB® includes two groups:
 

  • Women eligible only for family planning services.
  • Women eligible for care between pregnancies and for family planning services.
     

How Does Enrollment Work?

  • Georgia women who meet the eligibility requirements for the P4HB® program will be enrolled through the DCH Enrollment Process administered by the Georgia Department of Human Services.
  • Pregnant women enrolled in the Right from the Start (RSM) Medicaid Program will be able to enroll when their RSM eligibility ends.
  • Proof of continued income eligibility to DCH’s enrollment broker must be provided.
  • Eligible members will receive a P4HB® program membership card.
     

What Plan Options Are Available?

  • Family Planning.
  • Interpregnancy Care Services.
  • Resource Mothers Outreach.
     

Family Planning Services (Members are encouraged to seek care from a Federally Qualified Health Center or a Rural Health Center.)
These services include:
 

  • Medically necessary services.
  • Referrals as needed.
  • Education and counseling to make informed choices and understand contraceptive methods.
  • Complete physical exams including a pelvic exam and Pap test.
  • Follow-up visits.
  • Pregnancy testing.
  • Birth-control supplies and follow-up care.
  • Diagnosis of sexually transmitted infections (STIs).
  • Treatment of most STIs.
  • Tubal ligations (sterilizations) for some participants.
     

Interpregnancy Care Services

  • Immunizations.
  • Transportation.
  • Mental Health and Substance Abuse.
  • Case Management Services.
  • Dental Services.
  • Emergency Services.
  • Emergency Transportation Services.
  • Pharmacy.
     

Immunizations

  • P4HB® participants age 18: All vaccines under the Vaccines for Children (VFC) Program. Please talk about this with your PCP.
  • P4HB® participants ages 19 and 20 (as needed):
    • Hepatitis B (HepB).
    • Tetanus-diphtheria (Td).
    • Tetanus-diphtheria and acellular pertussis (Tdap).

Mental Health and Substance Abuse
A referral is required. Call WellCare of Georgia at 1-866-231-1821 with questions or to find a provider in your area. TTY/TDD users, call 1-877-247-6272.

Case Management Services

  • Early identification of special needs.
  • Assessment of risk factors.
  • Care planning.
  • Referrals and help to ensure timely access to care.
  • Coordination of care with providers in and out of network.
  • Resource Mothers Outreach.
     

Dental Services

  • Two exams per benefit year.
  • X-rays once a year.
  • Two cleanings per benefit year.

Emergency Services
Related to family planning only. For example, a complication after a tubal ligation.

Emergency Transportation Services
Related to family planning only.

Pharmacy

  • Folic acid and/or a multi-vitamin with folic acid.
  • Education and counseling needed to make informed choices and understand birth-control methods.

Resource Mothers Outreach
You may also be eligible for extra support through our Resource Mothers Outreach. The Resource Mother provides a broad range of services. Some of these are:
 

  • Counseling.
  • Help with personal and social problems.
  • Nutrition guidance.
  • Help to quit smoking.
  • Help with medical appointments for you and your baby.
  • Emotional support following substance abuse treatment.
  • Mentoring.
