Dear Provider,

Effective June 1, 2011, Avesis Third Party Administrators (Avesis) will manage all ophthalmology services for WellCare's Georgia Families members. Please note that Avesis already manages all optometry services for Georgia Families members. Avesis is a national vision company that contracts with optometrists and ophthalmologists to provide members the freedom to choose from thousands of providers in a nationwide network.

If you are an ophthalmologist, you may have recently received a letter from WellCare outlining changes to your provider agreement. If you do not intend to contract directly with Avesis to service WellCare members, and have concerns regarding the continuity of care for members receiving an active course of treatment for an acute episode of a chronic health condition, please call Avesis at 1-800-522-0258, ext 130 or ext 293, to coordinate a transition plan for those members. In order to ensure continuity of care, Avesis may authorize your continuation of services to WellCare members after 5/31/11.
